Quality begins with authority, not inspection at the end

Every engagement records the intended outcome, scope, exclusions, assumptions, constraints, data and environment boundaries, decision owners, specialist authorities, dependencies, acceptance evidence, escalation route, continuity expectation, and handoff owner. A principal owns the engagement structure while clinical, legal, accounting, privacy, security, regulatory, and client executive authorities retain their decisions.

  • Role charter: every named contributor receives the purpose, decisions, context, systems, data boundary, deliverables, acceptance evidence, escalation path, and prohibited actions before consequential output begins.

  • Assignment provenance: the record identifies who selected each contributor, the evidence supporting the assignment, who approved it, the scope, and every substitution.

  • Review plan: work receives the technical, domain, privacy, security, clinical, financial, legal, or release review required by its risk.

  • Decision log: material choices retain facts, assumptions, options, owner, rationale, evidence, date, and reopen trigger.

  • Evidence gate: deliverables point to reproducible checks and owned exceptions before acceptance.

  • Change control: material changes to scope, roles, data, environment, authority, or acceptance are approved and versioned before becoming the baseline.

Acceptance is a named decision

The work plan identifies what requires independent review, what each reviewer may see, what constitutes a material defect, and who can accept or reject the result. Original work, review comments, corrections, exceptions, and final acceptance remain attributable. High-consequence work preserves separation between the author and the required approving authority.

Exceptions remain visible until disposition

A material risk, decision delay, acceptance failure, access or data concern, safety issue, scope breach, conflict, or dependency failure becomes an owned record with condition, consequence, options, recommendation, decision deadline, and next communication. Corrective action records containment, supported cause, correction, systemic change, verification, residual risk, and owner.

Handoff transfers context, not merely files

Continuity combines named backups where appropriate, current context, decision records, artifact locations, access ownership, versioned procedures, unresolved risks, recovery paths, and a receiving owner. Handoff closes when the receiving owner can operate, challenge, and recover the work.
