A professional may report a material payment-record issue, assessment-process concern, conflict, discrimination or retaliation concern, access or security problem, unsafe or materially out-of-scope instruction, confidentiality or data concern, harassment, quality-process defect, misattributed work, inaccurate record, deactivation, termination, or other engagement-process issue.

Urgent safety, security, privacy, fraud, or access concerns follow the applicable immediate escalation path and are not left in a general queue.

The record identifies the issue, affected engagement or process, evidence, requested correction, assigned owner, conflicts, immediate containment, status, decision, and communication. A person materially implicated does not close the concern alone. Good-faith reporting does not alter a Rigors score or create retaliation in assessment decisions.

A current deactivation, suspension, or termination record states the applicable agreement or policy basis, evidence, decision owner, effective condition, access and data actions, payment and deliverable closeout, handoff, notice, and any review path the actual terms provide. Allegations remain distinct from findings, and corrections append rather than erase the original record.

The operating procedure and executed agreement govern response ownership, available remedies, continuation, reassignment, compensation, and escalation. A request can be upheld, corrected, reassigned, remediated, referred to another process, or closed with a recorded reason.
